If you prefer the government portal: go to turkiye.gov.tr, search for 'Turkcell Abonelik Fesih Başvurusu', click 'Kimliğimi Şimdi Doğrula' (Verify My Identity Now), authenticate via internet banking or mobile signature, select your line, click 'Fesih Başvurusu Yap', then download, sign, and upload the termination document.
