Review your contract terms

Go to https://www.t-mobile.cz/podpora/smlouva/jak-opustit-t-mobile and review your contract type. Check if you have a fixed-term commitment and whether you received a subsidised device, as you may need to repay the remaining device value. If you have been a customer for 3+ months, no penalty applies for early termination.

Download the termination form

Visit https://www.t-mobile.cz/ke-stazeni and download the contract termination form ('Žádost o ukončení smlouvy'). Alternatively, you can write a free-form written request or use the online contact form at https://www.t-mobile.cz/podpora/kontaktujte-nas — choose topic 'Smlouva a účet' then 'Ukončení smlouvy'.

Complete the termination request

Fill in all required details: your full name (jméno majitele smlouvy), the phone number or service ID to terminate (ukončované telefonní číslo nebo číslo služby), whether it is an early termination (předčasné ukončení), and your handwritten signature (vlastnoruční podpis). For the online contact form, type these details in the message body.

Submit the request

Choose a submission method: (A) Online — submit via the contact form at https://www.t-mobile.cz/podpora/kontaktujte-nas; (B) In-person — bring the signed form to any T-Mobile prodejna (store); (C) By post — mail to: T-Mobile a.s., Tomíčkova 2144/1, 148 00 Praha 4. Keep a copy or confirmation number.

30-day notice period begins

The 30-day notice period ('výpovědní doba') starts from the date T-Mobile receives your request ('ode dne doručení výpovědi'). Your services remain active during this period. For fixed-term contracts still within commitment, the contract ends either at the commitment end date or after 30 days if early termination was requested.

Receive final bill

Your last invoice ('Poslední Vyúčtování') arrives on your normal billing date, with charges prorated through the termination date. If you had a subsidised device, the remaining device cost is included. If porting your number to a new provider, obtain your OKU code (14-digit Ověřovací Kód Účastníka) from customer service 800 73 73 73 — the new provider handles the rest.
